  • Transcript from previous school. An official copy will be needed but we can work off of an unofficial copy if that is all that you have at the time of enrollment. Please request that an official copy be sent to Boonville High School, 300 North First Street, Boonville, IN 47601. A copy can be faxed to 812-897-6056
  • Medical and immunization records

Students entering Boonville High School must live in the Boonville High School district. If you are new to the area, you can verify your school district by contacting Warrick County School Corporation at (812) 897-0400. Call the school for an appointment at 812-897-4701 and ask for Mrs. Lashley.
